We, or I, hereby request that you accept the application for enrollment of ___________________________ in Total Soccer Training (TST) during the dates set forth in this application, and in consideration of your acceptance of the application we, or I, hereby release TST, John Burroughs School where TST will be conducted and all their respective staff and agents (together with TST), from all claims on account of any injuries which may be sustained by our, or my, child while attending TST and covenant not to sue TST for any injuries sustained and I indemnify and hold TST blameless for any claims which may hereafter be brought by our, or my, child as a result of any such injuries or death.
